THERAPEUTIC POSSIBILITIES IN DISTAL HUMERUS FRACTURES – CLASSIFICATION AO (ARBEITSGEMEINSCHAFT FÜR OSTEOSYNTHESEFRAGEN)
Abstract:
The objective of this study was the identification of the therapeutic possibilities for the
fractures of the distal humerus type B3.3 – classification AO. In a series of 773 cases of distal humerus
fractures, only 5 cases were identified with this type of fracture. In the treatment of this type of fracture
the following methods were taken into consideration: open reduction and internal fixation with Herbert
screws in 3 cases; arthroplastic resection in one case and a conservatory treatment (upon request) in
one case – this latter case was subsequently excluded for failure to show up. Given the very small
number of cases recorded, no solid conclusions could be drawn from the results obtained. However, the
data support the following assertions: the arthroplastic resection remains a solution to consider when
the possibilities of fixation are out of the question, the great inconvenient of this procedure being the
residual instability; the internal fixation with Herbert screws is the method of choice because the
fixation with plate according to the AO recommendations is often impracticable.
